Healthwatch Walsall is the independent local champion for health and social care services in Walsall. One of the ways that we do that is to develop an annual work plan based on the views of the public for improvements in health and social care. This survey will help us find out what your priorities are for health and social care in Walsall. Your information will be anonymised. Your information is important, and we must ensure that it is used with your consent for a legitimate purpose, securely stored and is not passed on to other organisations without your permission in relation to Data Protection. We will then create an annual work plan based on everyone's information with the aim of improving services. lf you agree to participate in this survey you will need to tick the box below.

Healthwatch Walsall is part of Engaging Communities Solutions (ECS). ECS is both the Data Controller and Data Processor in this instance. Under the General Data Protection Regulation (GDPR), the lawful basis we rely on for processing this information is your consent. By completing and submitting the survey you are consenting to us using it. You are able to remove your consent at any time. You can do this by contacting Healthwatch Walsall on the details below.
The survey will ask what your priorities priorities for improvements in health and social care services in Walsall are. This may lead to you sharing information about your/their health needs/experiences. The information will be anonymised to remove any personally identifiable information. We assign a unique identifier to it in case you/they request us to stop using it.

We will share the workplan with Walsall Metropolitan Borough Council, key health commissioners/providers in Walsall, Healthwatch England, NHS England and the Care Quality Commission as well as being published on the Healthwatch Walsall website for the wider public.
As part of the survey we ask for some information about the person completing the survey, but they do not have to give us this information to take part in the survey. We will ask for information such as their age, ethnicity, gender and whether they consider themselves to have a disability. We use this to ensure that we are reaching all parts of the Walsall community. |
Your individual information will be kept confidential and any feedback you give us will be anonymous. We will not share your details with anyone unless there is a legal reason to do so.
We keep your name and unique identifier separately to your anonymised information, securely in our offices until it is digitised, then held securely on our cloud storage. Information on paper will be destroyed by cross shredding. Any information we hold on you will be destroyed three months after we share our workplan with the Council. |
