Healthwatch Walsall have been commissioned by Walsall NHS Healthcare Trust and its partners to support them in delivering a programme of change in health and social care by making sure providers know what the views are of people in Walsall using health and social care services. The programme is called Walsall Together.

End of life care can be a very emotive and sensitive subject to talk about. The quality and accessibility of end of life care will affect all of us and it must be made consistently better for all of us. Walsall has developed an End of Life Strategy 2018-2023. Part of this strategy is to ensure that family members and those close to someone who has died are given the opportunity to share their views on the care that has been received.

This survey will give family members and loved ones the opportunity to help shape palliative care and end of life services in Walsall. Your views are really important.
