Healthwatch Walsall is the independent local champion for health care and social care services in Walsall. Part of our role is to understand the experiences of people who are using these services and to give people an opportunity to speak up and have their voices heard. On this occasion we would like to understand young people's thoughts on 'mental health' and the services that are in place, and in order to do this we are using this survey that has been developed by Healthwatch Central West London.

Your feedback is anonymous. Your information is important and we must ensure that it is used appropriately, secured safely and it not passed on to other organisations without your permission. We will use your anonymous information with that from other people to write a report that is shared with mental health service commissioners and providers, such as Clinical Commissioning Groups, NHS Trusts and Local Authorities. It will also be published on the Healthwatch Walsall website.
