Healthwatch Sandwell- Young People's Health and Social Care Survey
Healthwatch Sandwell is the independent local champion for health and social care services in Sandwell. Part of our role is to understand the experiences of people who are using these services and to give people a chance to speak up and have their voices heard.
We would like to find out your views about health and social care services in Sandwell. Your information will be anonymised. Your information is important and we must ensure that it is used appropriately, is secured safely, is not passed onto other organisations without your permission. Healthwatch Sandwell is part of Engaging Community Solutions. Engaging Community Solutions are both the Data Controller and Data Processor in this instance. Simon Fogell is the Data Protection Officer.
Under the General Data Protection Regulations (GDPR), the lawful basis we rely on for processing this information is your consent. You are able to reove your consent at any time. You an do this by contacting Healthwatch Sandwell on the details below.
This project will ask you for your knowledge and experience of using health and social care services. This may lead to you sharing information about your health and care needs. The information will be anonymised in order to remove any personally identifiable information. We assign a unique identifier to it in case you request us to stop using it. We use the information we collect to develop a report looking at all aspects of people's experiences of health and social care services and recommend actions to improve the service. We will share our report with Sandwell Council and publish it on our website.
As part of this project we ask for some information about you, but you do not have to give us this information to take part in the project.
Your individual information will be kept confidential and any feedback that you give us will be anonymised. We will not share your individual details with anyone else unless there is a legal reason for us to do so.
We keep your name and unique identifier separately to your anonymised information, securely in our office until it is digitised, then held securely on our cloud storage. Information on paper will be destroyed by cross shredding. Digitised information will be deleted.
Any information that we hold about you will be destroyed three months after we share our report with Sandwell Council.

Your right of access: You have the right to ask us for copies of your personal information.
Your right to rectification: You have the right to ask us to rectify information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

Your right to restriction of processing: You have the right to ask us to restrict the processing of your information in certain circumstances.
Your right to object to processing: You have the right to object to the processing of your personal information in cerain circumstances.
Your right to data portability: You have the right to ask that we transfer the information you gave us to another organisation, or you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
