Experiences of Patient Care Pathways- Living with a long term condition
Healthwatch Walsall is the independent local champion for health and social care services in Walsall. Part of our role is understand the experiences of people who are using these services and to give people an opportunity to speak up and have their voices heard.
Healthwatch Walsall have been commissioned by Walsall NHS Healthcare Trust and its partners to support them in delivering a programme of change in health and social care by making sure providers know what the views are of people in Walsall about using health and social care services. The programme is called Walsall Together.
We would like to find out your views about how people are supported and how people look after themselves living with a long term health condition. These may include diabetes, respiratory illnesses, cardio or heart conditions, mental health services, and people who have more than one long term condition. We would also like to speak to patients, carers and relatives about end of life services in Walsall.
This survey will help us find out what keeps you as well as possible, plus how you are supported, cared for and which services you use. Your information will be anonymised. Your information is important and we must ensure that it is used appropriately, is secured safely, is not passed on to other organisations without your permission.
